 Homeowner Assistance Fund (HOAF)

​​​Required Documentation​

  1. Copy of state-issued photo ID – must not be expired.
  2. Documentation of all income currently receiving:
    1. Current pay stub
    2. Year-to-Date Profit & Loss Statement if self-employed
    3. Unemployment
    4. VA benefits
    5. SSI/SSDI
    6. Child support
  3. Most recent statements for assistance requested:
    1. Mortgage statement(s) reflecting applicant name(s), property address, PITI, total arrearages, if any, total amount due, outstanding balance
    2. Past due utility bill. Minimum $300 required.
    3. Past due homeowner's association bill.
    4. Past due city / county property tax bill.
    5. Partial Claim or Payment Deferral Agreement if any
  4. Copies of complete bank statements, most recent, all pages, all accounts (checking, savings, Venmo, Cash App, etc.).
  5. Deed – proof of ownership.
  6.  Mortgage origination – original mortgage, note, closing disclosure, etc.
  7. Property Value (PVA statement or current tax bill reflecting value)​

The Housing Counselor may ask for additional documentation not included on this list, if needed.
